Registration Information (Lea en español a)

The following things are needed when enrolling a student:

1. Parent (or legal, court-ordered guardian)must accompany the student.
2. Student and parent must live within the GPHS school zone. Legal residentcy is established by both of the following :
	1. A RECENT UTILITY BILL (electric gas OR water) or lease agreement
	2. AND DRIVER'S LICENSE showing the parent's name and current address
		If the student and parent live with a friend or relative, legal residence is established with the following:
		1. parent's driver's license with the correct address
		2. and the friend's or relative's utility bill
		3. and a notarized letter stating that the student and parent live with the friend or relativeat the address (form letter available in the main office)
		4. OR parent's driver's license and lease agreement with student's and paren't name listed.
3. Birth Certificate or legal document with birth date and full legal name.
4. Student's social security card and/or number. (The disclosure of a social security number is voluntary unless you are informed otherwise. Your social security number will be used by the GPISD as a unique reference for its owner to promore accuracy of records, to facilitate record keeping, and to enhance statistical analysis. Social Security numbers will not be made available to the general public.
5. Students's current immunization records.
6. Previous school records (i.e. transcript, report card, & withdrawal papers from the previous school). These should be complete with no outstanding obligations. They are used to determine class placement. Prior disciplinary records will be considered for proper school placement within the Grand Prairie Independant School District.

All GPHS enrollment information must be completed with
verifiable phone numbers and addresses.

PROCEDIMIENTOS DE REGISTRO

Se necisitan los próximos documentos para registrarse:

1. Certificación de residencia (escritura de arrendamiento de adquisición, cuenta de luz ó de agua, tec...)
Si no viven en su propia residendia:
	1.Se requiere una declaración jurada firmada por el dueño del domicillo y por un notario. La declaración jurada se puede obtener en la oficina principal antes de registrarse.
	2. Le declaración jurada será acompañada por una cuenta del luz or de agua.
2. Acto de nacimiento/licensia de conducir
3. Tarjeta de seguro social
4. Recor de inmunizaciones
5. Transunto ó calcificatión final
6. Paples de retiro de la escuela anterior--se requieren para asignar clases apropiadas.
7. La forma de registro de GPHS necesita tener número de teléfono y dirección que se puede verigicar.
El estudiante sera acompañado porsu padre ó su guardián legal** para poder inscribirse. Es la poóliza del disyrito escolar que estudiantes quien viven con sus padres ó gurariandes legales ó otras personal con autoridad ytransgerido por tribunal de justicia quienes vbiven dentro los limites del distrito escolar de Grand Prairie podrán inscribirse.
